Doorstep Connections Programme
A caring, at-home mental health support service for people finding daily tasks overwhelming. We understand that getting to the GP, catching a bus, or simply going outside can feel impossible when your mental health is low. Many become increasingly isolated. With Doorstep Connections, we come to you, helping build confidence through small, achievable steps.
What is the Programme about?
We meet you where you are. From the comfort of your home, we offer gentle support to help you reconnect with everyday life when things feel too much. For some people, even getting to an appointment, using public transport or joining a group can feel overwhelming. Our aim is to make those first steps easier.
This is about taking small but important steps. We go at your pace, helping you rebuild confidence and independence. We use a simple step-by-step approach that helps you feel more comfortable in everyday situations.

What We Do
Before we start, one of our team will call you for a friendly chat.
We use this to learn more about what matters to you and how we can best support you.
Typical support includes:

An informal doorstep check-in or chat

Short accompanied walks near your home

Exploring local community spaces or activities

Encouragement to reconnect with hobbies, friends and daily life
Our focus is practical, social and confidence-building.
You decide what feels comfortable, and we guide you through each stage at a pace that suits you.
Our Approach
Everything we do is centred on you. We believe in doing things with you, not to you.
Our approach combines lived experience, compassion and practical wellbeing methods to help you move forward.
For some participants, we may link in with one of our independent counsellors or therapists if extra emotional support would help. This makes sure anyone needing additional one-to-one support can access it safely and separately, while keeping the programme focused on everyday wellbeing.
How It Works
- Referral – You can be referred by your GP, social prescribing link worker or community group, or you can refer yourself.

Introductory Call – We will arrange a time to talk about what you need and explain how the programme works.

Step-by-Step Support – We begin with small actions such as short walks, trying public transport or visiting local groups together.

Building Confidence – We review how things are going and build on what has worked for you.

Next Steps – If you would benefit from more in-depth support, we can introduce you to trusted independent practitioners who provide that separately.
Why It Helps
Doorstep Connections helps reduce isolation and rebuild everyday confidence. It gives you space to practise small changes in a supportive, real-world way. Participants often tell us they feel more comfortable leaving the house, reconnecting with others and managing daily routines.
How to Access Support

Professionals: GPs, social prescribers and community workers can refer directly using our Professional Referral Form.

Self-Referrals: Individuals can use the Self-Referral Form.

We respond to all referrals within 48 hours.
If, during your time with us, you feel you would benefit from talking to someone one-to-one, we can connect you with a trusted independent counsellor or therapist through our separate Private Therapy and Counselling route.
