Doorstep Connections Programme
A caring, at-home mental health support service for people finding daily tasks overwhelming. We understand that getting to the GP, catching a bus, or simply going outside can feel impossible when your mental health is low. Many become increasingly isolated. With Doorstep Connections, we come to you—helping build confidence through small, achievable steps.
What is the Programme about?
We meet you where you are. From your doorstep, we ease the burden of daily logistics and support you in reconnecting with life when it feels daunting. This programme is about taking small but important steps at a pace that feels right for you. We accompany you on short walks, help you use public transport and encourage gentle outdoor activity. Sessions build on each other, steadily increasing confidence through a mix of therapy methods, including Graded Exposure.
“Graded exposure involves exposing you to the source of your fear gradually by going up the ladder one step at a time”
Graded exposure is widely used in exposure therapy: it helps reduce anxiety and avoidance by introducing manageable challenges and building resilience over time.
What We Do
-
Before any sessions begin, one of our team will be in touch to introduce themselves, find out more about you and gather any necessary information
- We’ll arrange a convenient time to call and explain how the programme works
- We visit your doorstep for a relaxed, friendly chat at a time that suits you
- We accompany you on short walks, using public transport or spending time outdoors; whatever feels manageable
- Each session builds on the previous one, helping you to grow confidence steadily through techniques such as graded exposure and other proven therapeutic methods
- We follow up with brief visits after two and six weeks to see how you’re progressing and offer continued support
Our Approach
-
We work with you, not impose on you.
-
Conversations are relaxed, and fully shaped by your needs.
-
We follow your pace, whether that’s getting outdoors, trying public transport, or regular doorstep visits.
-
We use social prescribing, an approach shown to reduce isolation and boost wellness through community‑based support and activities
Elements of the Programme
- Initial Visit – informal doorstep chat to explore how you’re feeling and what support you might like.

- Written Summary – clear next steps and helpful contact details.
- Check‑In Visits – visits at two and six weeks to support continued progress.
