Client Self‑Referral Form
Who is this page for?
This page is for individuals who want to refer themselves into one of our Community Programmes, without requiring a doctor or professional to refer you. If you are experiencing mental health challenges (such as anxiety, low mood, isolation or lack of confidence in daily life) and would like supportive, community‑based care from Mind Matters NE, this is for you.
What Happens After You Self‑Refer

We aim to respond within 48 hours

A member of our team will give you a call to introduce themselves, get to know your situation, and explain how the service works

Together we match you to the programme(s) that suit your needs best

We’ll confirm your place and send details of your first session or activity
Information We Ask You to Provide
The self‑referral form will request:

Your full name, date of birth and contact details

Your address (including postcode)
-
Preferred method of contact (phone, email etc.)

A brief summary of how you’re feeling and what support you hope to receive

Your GP practice (if relevant)
-
A declaration that you agree for us to use the information you share with us
Professional Referrals Still Welcome
If you’d prefer to be referred by a GP, social prescribing link worker or community group, they can use our Professional Referral Form. We accept these referrals and handle them in the same way, responding within 48 hours.
